Initial Comments:
Based on the findings of an onsite unannounced follow-up survey completed August 23, 2019, Community Care Inc. had corrected the deficiencies cited under PA Code, Title 28, Health and Safety, Part IV, Health Facilities, Subpart A, Chapter 51. The deficiencies were cited as a result of a state re-licensure survey completed May 9, 2019.
